SNAP

EIS Screener

Ardmore City Schools will be providing the Early Identification System (EIS) screener within the school year to aid the district in determining student needs and providing prevention programs. The screener questions are available for viewing below. Screening for Kindergarten through 4th grade students will be completed by staff, while screening for 5th through 12th grade students will be completed by the student.

Students whose parents provided permission/consent during the enrollment process will take part in the screener.  You may choose to withdraw your child from the screening at any time. This is NOT a psychological evaluation, and the information will NOT be included in your child’s permanent academic records.
